To become an exhibitor, click to download the Exhibitor Application and the Exhibitor Agreement and fill them out. You can then email a signed copy to marketing@thebackdoctors.com, fax to (801) 225-2537 or mail to 218 East 800 South Orem, UT 84058. You will also need to download the Expectations form and the Tips document. We would also like you to email your logo to us to be used in advertisements for the health fairs. We will coordinate everything with you to make sure you have success at the health fairs and help you to provide a benefit to the attendees.
